Frequently asked questions
How it works
GP referral
Your GP will create a referral and send it to the Lincolnshire EACH Team who will contact you to refer you to Community Healthcare. You will then receive an E- referral letter with your booking reference number and our contact details.
Ring us to book your appointment
We'll take your details and go through our pre-op consent with you and arrange a convenient appointment time and location.
Confirmation letter
You'll receive a confirmation letter with details of your surgical address, timings and any relevant instructions you may be required to follow.
Don't forget, you can refer yourself for any LARC procedure!
